Important Notice About Forms 1095-B and 1095-C

Under current IRS regulations, employers are not required to automatically furnish Forms 1095-B and 1095-C to covered individuals unless a copy is requested.

New Haven Community Schools will provide Forms 1095-B and/or 1095-C upon request. These forms contain information about health coverage offered or provided during the prior calendar year and may be used for personal records or tax preparation.
